You can request credit for an individual or a group right here. If you are requesting credit for an individual, use form 725. If you have a group that have used a course together, use form 775. However, if you use form 775, all of the participants must already have completed work in the CGSP system. Use form 725 for all first-time participants.

Fill out the information on the form, then click on Submit form. It will be emailed to the CGSP staff.

Remember, if the course was less than 10 weeks in duration, you will not receive a certificate for that course alone. You will receive credit only which will be applied to certificate plan(s). If the course was 10 weeks or more, you will receive a certificate in a few weeks. It will be mailed to your church office if your CGSP file has a church identification. Otherwise, it will be mailed to your home.
